Service Related Questions
The Ahero programs are all customized based on the needs of the patient, the digital and telehealth options available to effectively assist, and then the most appropriate resources available. They are not standardised treatment programs. The Ahero assessments are also evidence-based and validated outcome measurement tools, so our assessments and results are data-driven and offered with clear targets and feedback that help the patient stay in control.
If you work or collaborate with the Ahero program, you would likely work with one dedicated group of professionals so that you can all learn, grow and help together. Ahero’s team are big believers in the power of the network, and the importance of depthful, longstanding relationships. We all work better with people we get to know over time, and just as you develop a relationship with your patients, so too should you develop ones with your health team.
We do NOT interfere with OHIP or any provincial payor system. We have no Access Bonus issues, and we are completely on-side with the OHIP fee schedule and applicable laws surrounding the compensation of physicians.
Referring is easy, just click here to download our referral form, or invite your patient to contact us directly.
When one of your patients enrolls with Ahero, we notify you via fax and request their chart so our NP’s can review and ensure the right information in their chart flows to the right people on their health team. We invite you to patriciate in the program and join the NP and allied health providers for a 5 – 8 minute encounter for case management and alignment, and when the patient needs something or calls the NP for support, our NP would confirm with you or send you their notes so you’re constantly in the loop and able to lead medical decisions. If you decide not to participate, you will NOT lose the patient from a rostering perspective. You will receive a copy of your patient’s ACTIVE report, and be kept informed of clinical encounter along their journey with us.
